First let's look at what we need to know in theĀ anatomy and the structure. So there are two kidneysĀ Ā
in the abdomen and each kidney has millions ofĀ nephrons. The kidneys have an excretory portionĀ Ā
and a collecting portion. The excretory portionĀ of the nephron starts from the glomerulus, thenĀ Ā
the proximal convoluted tubule, the loop of Henle,Ā and the distal convoluted tubule. The collectingĀ Ā
system is the collecting ducts and tubules, whichĀ form the minor calyces, then the major calyces,Ā Ā
the renal pelvis, which continues down as theĀ ureter, which is going to empty into the bladder.
The kidneys develop from the intermediateĀ mesoderm and the portions of this mesodermĀ Ā
that condense to give rise to the kidneysĀ are the nephrogenic cords. There are two kidneys,Ā Ā
so they're bilateral. They extend from theĀ cervical end to the sacral end of the embryo.Ā Ā
So if we assume this is the back of the embryo,Ā it's from the neck end up to the sacral end.Ā Ā
There are three stages of development. The pronephros,Ā then the mesonephros, and the metanephros.Ā Ā
They're one below the other. So the pronephrosĀ is at the cervical region, the mesonephrosĀ Ā
is between the thoracic and the lumbar regionĀ and the metanephros is lower lumbar and sacral.Ā Ā
The first stage is the pronephros. We'reĀ now at week four. The pronephros is veryĀ Ā
primitive, just some cell clusters and tubules,Ā and the pronephric duct, this duct opens intoĀ Ā
the cloaca, part of which is going to form theĀ bladder. But for now it's still the cloaca.Ā Ā
So the pronephros can't function, itĀ degenerates. So we're just going to remove that
and now we're on to stage two, the mesonephros.Ā Ā
This appears late in the fourth week. They haveĀ mesonephric tubules, which are like primitiveĀ Ā
versions of renal tubules, and these are associatedĀ with capillaries. What's that? That's a glomerulus,Ā Ā
that means they can perform filtration,Ā and they drain into the mesonephric duct,Ā Ā
also called the Wolffian duct. Again that opens intoĀ the cloaca. So this stage can perform filtration, soĀ Ā
it's functional, and it does the work of theĀ kidneys, until the final kidney takes over.Ā Ā
So up to around 12 weeks. These tubules, theyĀ start degenerating as well, from the top down.
By the fifth week, the metanephros forms.Ā Now this is going to form the actual kidney.Ā Ā
It starts functioning towards the 12thĀ week, it takes over from the mesonephros.Ā Ā
From the lower portion of theĀ mesonephric duct, there's a bud that grows.Ā Ā
This is the ureteric bud. Now this is goingĀ to form the collecting system of the kidney.Ā Ā
The lower part of the nephrogenic cord has theĀ metanephric blastema. The ureteric bud inducesĀ Ā
differentiation of the mesoderm in this region,Ā and that's going to form the excretory portion.Ā Ā
First let's look at the ureteric bud. The caudalĀ part of the bud, that's going to form the ureter.Ā Ā
So that leaves the cranial part to form the rest.Ā So it branches and forms those collecting tubules.Ā Ā
The first few set of branches will form the majorĀ calyces and the next set will form the minorĀ Ā
calyces. So we're done with the collecting system.Ā We've got the calyces, the pelvis, and the ureter.Ā Ā
Now on top of each of these collecting tubules, theĀ metanephric mesenchyme forms a cap. This is goingĀ Ā
to form the excretory portion of the kidney. TheĀ ureteric bud induces differentiation of this cap.Ā Ā
There's formation of a renal or a metanephricĀ vesicle. So these vesicles, they become S-shaped,Ā Ā
and one portion of this S gets associated withĀ capillaries. So that's going to form the glomerulus.
The rest of the S becomes the renal tubule. TheyĀ lengthen and they form the proximal convolutedĀ Ā
tubule, the loop of Henle, and the distal convolutedĀ tubule. So we finally got a nephron. So the uretericĀ Ā
bud induces the metanephric blastema, and that inĀ turn induces the ureteric bud. So the ureteric budĀ Ā
forms the calyces, the renal pelvis, and the ureter.Ā So everything from the collecting duct downwards.Ā Ā
And the metanephric blastema forms theĀ excretory portion, from the glomerulus toĀ Ā
the distal convoluted tubule. As gestationĀ continues, the number of nephrons increase,Ā Ā
because ultimately the kidneys have millions ofĀ nephrons, remember? So the kidneys have developed inĀ Ā
the pelvic region, but the kidneys ultimately areĀ going to be in the abdomen. And that's because withĀ Ā
body growth, they ascend. So initially their bloodĀ supply is from branches of the abdominal aorta,Ā Ā
but finally the renal arteries comeĀ directly off the abdominal aorta.
So during foetal life, when the kidneys function,Ā they form urine. That drains into the cloacaĀ Ā
and then into the amniotic fluid. And thatĀ is how the kidneys develop. Three stagesĀ Ā
