mange måter det er der hovedtyngden
ligger eh når det gjelder å utjevne
sosiale helseforskjellene og også å få
til denne helhetlige

og og det som vi kan
eh og det som er helt nytt altså som er
viktig prinsipp er at vi har hatt lover
før som har lagt ansvaret på kommunen
for helhetlig få et helhetlig

kommunen skal ha oversikt over

politikere andre politikere De har ikke
alltid den samme agenda som
folkehelseforeningen
i alle de nordiske land er ikke bare
basert på en topp Down styring gjennom
lover det er også en rekke kommuner har
også en veldig høy grad av
selvstendighet til selv å prioritere og
i Norge henger det veldig klart eller i
alle de nordiske landene og som i
finansieringsordningen kommunene får en
Ram et rammetilskudd og så prioriterer
de der og det har jo gjort at staten
også må ha noen andre virkemidler i
forhold til
kommunene og penger har jo vært et
virkemiddel mye til å stimulere til å
utvikle nye områder men innenfor
folkehelsearbeidet kan vi si at det
følger lite eller ingen penger med eh Et
annet poeng som er litt viktig å si når
det gjelder dette med flernivå styring
eller multilevel governance som det
heter på engelsk er også at hele
reformen er jo iverksatt innenfor en
sånn new public management tankegang
hvor det er en sånn her målstruktur som
ligger fast og for eksempel Når det
gjelder dette med å utvikle en helhetlig
politikk tvers sektoriell
samarbeid så kan det ofte krasje med eh
andre mål bå det dette med tidsbruk
økonomisk effektiv
og så
videre så dette med at staten har noen
styrings virkemidler gjennom planverket
er en ting men kommune stat kommuner
relasjonen innebærer mye mer enn
det det andre temaet vårt helse i alt vi
gjør er dette med at vi skal fokusere på
det tvers Sektor samarbeidet for det har
jo vært et honnørord i veldig mange år
men ofte vanskelig å få til og det
kanskje ofte staten mer enn kommunene
sin skyld for staten og gjennom
departementene har jo en veldig klar Sil
og struktur og har også en en struktur
overfor kommunene som er veldig Sektor
isert som hvor kommunene får en masse
insentiver fra staten men så skal de
allikevel forsøke å få en og kanskje
disse brevene som staten sender til
kommunene så står det til slutt Ja og så
må vi ha litt mer tekt
samarbeid så det er jo også en det
tredje er jo dette med de kommunale
tjenestene hvordan de skal da få til å
eh å kunne koordinere seg innenfor både
det kommunale eh virkeligheten som ofte
handler mer om å skjære ned enn å å
legge til rette for økt eh eh for økt eh
samarbeid så i dette prosjektet så
eh e har vi hat to
ehm undersøkelser en med spørreskjema
til alle kommunene i 2011 rett før
prosjektet startet og en nå i vår og vi
har også hatt intervjuundersøkelser i en
rekke kommuner nå i
vår så jeg skal først presentere lite
grann fra en artikkel som vi skrev fra
den baseline undersøkelsen om hvordan
kommunene håndterte
eh denne denne her de insentivene kom
dette var altså før loven trått i kraft
men vi hadde hatt disse insentivene i
flere år gjennom eh ulike
stortingsmeldinger og policy dokumenter
eh
Og Ja og det var en en
spørreskjemaundersøkelse til eh alle
kommunene som første og det som var i
2011 12 var jo at det var en ganske stor
forskjell mellom hva kommunene mener var
viktig Og hva staten kommunen er viktig
Det som er viktig i Norge er at
folkehelseloven er del av en større
reform som heter samhandlingsreformen
som også endrer på altså kommunene skal
ta en større ansvar for pasientene i som
ble utskrevet fra sykehus den reformen
følger det penger med så i kommunene i
hvert fall i 2011 så var det veldig mye
mer fokus på den delen enn den andre
delen og det var også der det var penger
eh og samtidig så var det at kommunene
hadde kanskje i Skal vi se Det var
den Ja At vi ser at det var en Nei nå
tøys jeg her Det var den Ja skal vi se
et må gå tilbake til den Ja at at dette
med levevaner og det som heter Friskliv
var på en måte Det som var i fokus
veldig mye da i
2011 og det var veldig få kommunene som
hadde fokus på på de sosiale
helsedans vi sendte ut en ny
spørreskjemaundersøkelse i vår og det
jeg må si at den har vi altså bare for
en to uker siden begynte å se på Så vi
har veldig Vi har ikke klart å lage
sammenlignende tall for de spørsmålene
var litt sånn annerledes men jeg skal si
litt om den nye mens hovedkonklusjonen i
den første var litt i 2011 var ganske
pessimistisk at noe av signalene som var
kommet fra de eh og det
stortingsmeldingen fra 2007 om utjevne
sosiale helseforskjeller og
fylkeskommunes rolle var var ganske Hva
skal vi si litt
pessimistiske så ser vi nå at det ser ut
som det er en Prosess på gang i hvert
fall på disse holdnings spørsmålene så
sier kommunen at de har endret Fokus at
det tverrsektorielle samarbeidet er
styrket og at det har skjedd en styrking
av kunnskapsgrunnlaget og at folkehelse
integrert i kommuneplanen
Men hvis vi spør mer konkret om hvem som
har utarbeidet en oversikt over
helsetilstanden og positive og negative
påvirkningsfaktorer så er det da under
40% som har gjort det allerede Og mens
nesten resten sier at det er en Prosess
til å starte Eh vi Spør også om
kommunene har etablert tvers aktuelle
samarbeidsgrupper og da ser vi at over
60% har gjort det nå en det som
forskningen virker til og ser ut til å
ha veldig stor betydning det er rollen
til
folkehelsekoordinatoren og ikke bare
folkehelsekoordinatoren veldig mye Hvor
folkehelsekoordinatoren er administrativ
plassert er det oppe rådmannen stab der
hvor planen utvikles og politikken
utvikles eller er det nede i
organisasjonen i helsetjenesten har de
en stor stillingsprosent eller kombinert
de det med andre for eksempel
helsetjeneste tiltak Eh vi ser det at
det er altså ganske mange flere eller
10% flere som har en
folkehelsekoordinator i dag enn det
hadde før men hvis vi ser på hvor de er
plassert så ser vi fortsatt at det er
helsetjenesten som har hvor det flest
som er plassert og at det er da bare
drøy fjerdedel som er
eh administrert hos Rådmann stap og få i
noen av de andre eh når det gjelder
sosial ulikhet i helse så svarer i grunn
de fleste at dette er et prioritert
område Men vi vet Ena ikke På hvilken
måte eller hvilken sånn tiltak eller åt
gjerder som som gjøres og så vi ser at
det aller mest når det er innenfor
folkehelse og forebyggende så er
innenfor
helsesektoren
Eh det for å oppsummere Så langt så kan
vi si at det synes å være en del
positive utviklingstrekk i kommunene Det
er Stadig flere kommuner som ansetter
folkehelserådgiver eller
folkehelsekoordinator og det tvers
Sektor alle samarbeidet synes å være
utvikling eh Men vi vet ikke hvor sterke
Disse føringene er og hvor forpliktende
dette samarbeidet er og på Hvilket Nivå
Det er det er noe vi skal begynne å
analysere nærmere eh loven har lagt
sterke føringer på kommunene om
integrering i kommune planen og Dette
ser det ut som kommunene tar veldig på
alvor De har også fått en del
hjelpemidler når det gjelder dette i
form av helse oversikter som de selv kan
finne ut av som Folkehelseinstituttet
har stilt rådighet og det vi lurer på om
vi kanskje er litt vanskelig å si med
det foreløpige analysene om dette her
samrøret mellom Friskliv og folkehelse
holder på å løse litt opp for dette som
heter Friskliv i Norge eh eh er altså et
en
helsetjeneste mens folkehelsen er jo det
som skal foregå i de andre
sektorene så det mener vi er en av de
viktige tingene vi må analysere nærmere
Vi har også De kvalitative dataene og
det vi ser at de kommunene som har
forstått dette De for de så er det helt
selvsagt at Friskliv og folkehelse er to
ulike
inngangs skal vi si porter til til
helse eh og det vi også tenker at dette
med at vi har dette sterke kommunale
selvstyret vil bety veldig mye at
kommunene vil utvikle seg ulikt Tror vi
foreløpig ser det ikke ut som at det er
de fra den første undersøkelsen de
kommunene med de største sosiale
ulikhetene som eh mest satser på
folkehelse det er ofte små kommuner
mindre kommuner hvor det er lett å få få
til et samarbeid som kanskje har en lang
tradisjon for det i i Norge så er det de
store byene hvor det er de største
sosiale skilnadene mens denne
forståelsen av at at sosial ulikhet i
hels med opp agender eh Den er ikke
alltid til
stede og så er det også til slutt tenker
jeg at det som kan bli veldig viktig det
er jo dette med at hvis det er sånn at
staten mener eller regjeringen mener at
folkehelse skal være et høyt pri tekt i
et eh ansvarsområde Hvor hovedmålet er å
utjevne sosiale helseforskjeller Da
tenker at vi trenger flere insitament
også overfor eh

også har kommet fram til nå sist og som
besøkt en kommune var at de er i gang og
de vil satse på folkehelse men det er
litt vanskelig for dem å å St å stokke
kortstokken Hvordan skal oss organisere
det Og hvordan skal oss gjøre det så jeg
tenker fra sånn fra litt sånn overfor
fra fylkesmann rolle og for myndighetene
sin rolle så tenkte jeg kanskje at en av
virkemidlene kan være på en måte
hjelpe litt kommunene hvordan de kan
organisere seg Fordi at det mye de vil
satse på folkehelse og de men også de f
blant annet ha tilsyn med de med
helseoversikt og de det er jo mange som
ikke har startet med den helseoversikt
sin men de har en god del oversikt over
sin helse og da kunne få organisert deg
opp i en helseoversikt som de kan da kan
bruke men også viser kommunen at de kan
bruke det det til så mye ikke bare til
det for oversikt over folkehelse og
folkehelse men også se det opp mot
dømmes økonomi og prioritering av tiltak
det tenker jeg at og tenker kommunene i
hvert fall sånn som oss har sett det må
få hjelp til å se den folkehelseloven
til det den er verd
og så jeg tenker at fra myndighets siden
Kanskje kan ta det inn i Trondheims
erklæring og at at oss som myndighet må
hjelpe kommunene til på en måte å se
folkehelse i et bredere perspektiv Da

veldig kort og det er når vi går inn i
disse kvalitative studiene våre i
kommunene så ser vi det At kommuner som
kanskje har jobbet med tilsvarende eh
prosjekter eller tiltak eller satsninger

eksempel sunne kommuner eller nærmiljø
prosjekt De er mye mer der de ser det
umiddelbart på sett og vis sånn at det
