call it varicocele repair there's
different types of surgical repair for
varicocele but the gold standard is a
sub inguinal microsurgical approach so a
small incision right where the pubic
hair grows so it's about a three-quarter
inch incision on either side so if it's
bilateral it'll be on both sides so we
shave the pubic area and that's where
the incision takes place so it's called
a sub inguinal incision and then we use
an operating microscope which is a big
big camera that magnifies down onto our
surgical site and what we do is we
identify the arteries veins and vas
deferens to the testicle once we've
identified the artery we then go through
from left to right all the way across
and find all of the veins we use a
Doppler ultrasound that helps us
identify the signal for the artery just
to be sure that every vein that we take
is is a vein and then once we've taken
all the veins we put the the spermatic
cord back where it belongs uh and um and
make it like we were never there so what
that does is it prevents the the
retrograde flow going down the the
testicle and The Rock towards the
testicle in the wrong direction now the
testicle actually is able to still get
its uh the blood that comes down through
the artery still able to get back uh
through the uh an alternative uh route
but it's not getting the retrograde flow
coming from the big what's called
gonadal Vein from up near the kidney and
what this does is essentially allows for
uh regulation of temperature so the
testicle has a more regulated uh
regulated environment for its
temperature for for adequates for
metagenesis
it takes about three to four months to
see the improvements and that's with any
with any intervention that we make so if
someone stops smoking stops smoking
marijuana or Cuts back on alcohol use or
we start a new medication for a hormonal
problem what that does is it it takes
about 90 days for the sperm to a change
and the reason for that is that
spermatogenesis from production to
ejaculation takes about 74 days and so
we round up and say that after about 90
days is when we start seeing a change
